2013 Extension Forum Evaluation – Judy Barth
The evaluation of the 2013 Extension Forum is now on line at: http://studentvoice.com/col/2013forumeval. This link will be active until midnight on Friday, December 6. Please provide input on this year's Forum and comment on the timing of next year's Forum by completing this survey.
Distinguished Alumni Awards
The deadline for nominations is April 14, 2014, and nominations are accepted at any time before that date. Nomination information including past recipients, award descriptions and nomination guidelines/forms may be found on the Alumni Association website at http://www.alumni.colostate.edu/Events/DistinguishedAlumniAwards/

Program Leadership Team (PLT) Update – Jan Carroll
Planning and Reporting Units (PRUs) begin meeting next week. These meetings focus on planning, based on local demand and University resources:
- What is the condition we want to affect?
- What are the behaviors we want to influence that will affect that condition?
- What education will we provide that will support the behavior and affect the condition?
- How will we evaluate/measure our success?
- What work (outputs) we will do in 2014?

You can find this form via our ‘Promotional Items for Sale’ web link (www.ext.colostate.edu/staffres/market.html)’ click on the link at the top of the page **New Extension Apparel & Accessories Online Ordering Form**.
This is the ordering form that was first release at Forum, and subsequently released through this email update venue. However, based on feedback since the release, we’ve added some features, including a sizing chart and instructions and charges for the process of adding your county or area name under the CSU Extension art.
