Colorado State University Extension has skilled and highly trained professionals (agents, specialists, and others) helping Colorado’s citizens, businesses, and communities apply unbiased, research-based knowledge. Extension’s Performance Appraisal Process is designed to recognize and support performance and leadership. It is designed to accommodate the unique and diverse position responsibilities with Extension.

The newest performance appraisal document, updated and finalized in December of 2018, encompasses the employee’s work during the prior calendar year. These changes are based on feedback gathered in 2017. This performance appraisal document is intended to focus on those aspects of the employee’s performance done well and areas for growth. Employees are to complete all sections in light green; supervisors will complete sections in teal. Please note that comment boxes are limited to the space shown. You are working within a fixed space with character limits; comment boxes will not expand. You may have to be selective when sharing information in order to accommodate the space provided.

Download the new performance appraisal document and save to your desktop or C: drive. Open in Adobe DC, Adobe Pro, or Adobe Reader. This dynamic form will work in all three platforms. You may encounter problems if you do not use Adobe.
